Two and a Half Minutes with Dr. Mark Spier

Your PinPointe FootLaser treatment just received clearance from the Food and Drug Administration. How long have you been seeking approval?

The application was submitted to the FDA in fall 2008.

You’re the first physician in Greater Baltimore to be a certified provider of this treatment. How many patients have you treated using this device?

We’ve treated almost 1,000 people with this device. It makes it easier now that it’s cleared to promote it. We’ve treated 70 to 75 people since receiving the clearance, but we’ve been using it in Maryland since 2009 off-label.

How do you get nail fungus?

Nail fungus is a condition that affects not only toenails but fingernails as well. It’s basically the same pathogen that causes athlete’s foot. The nail fungus gains entry to the nail so sometimes you can get it in a nail salon, such as by a pedicure. Guys typically pick it up in gyms, swimming pools and public showers.

How many people does this condition affect?

My best guess is about 10 percent of the population, so about 35 or 45 million Americans have this. People are embarrassed by it, so they’re not likely to talk about it.

What’s the alternative to a foot laser to treat fungus?

You used to take a drug by mouth to treat fungus. But the fungus gets established in the nail and it’s very hard to eradicate it by pills and topicals. Plus, many patients are reluctant to take the pill because it can affect the liver.
